Description: In this session, we will describe a collaborative interdisciplinary 22q team approach to the management of early health, feeding, speech/language, developmental, and psychosocial needs in infants and toddlers with 22qDS. This multidisciplinary session will include a nurse practitioner, who also serves as the primary care coordinator for the 22q team, feeding specialist, and speech-language pathologist from a 22q Center at a large pediatric hospital which uses a team care model adapted from the ‘traditional’ cleft team approach. The presenters will include a review of early 22q care needs, common feeding/swallowing and nutrition management practices, and early intervention therapy approaches utilized with this population. Successful strategies to ensure new patients receive necessary screenings, appropriate psychosocial supports, patient education and outreach strategies, and care coordination not only across the cleft team, but across the entire health system, will be discussed.
